1) How to Submit a Takedown Notice

Include all items below:

4. Your contact information (email, telephone number, and mailing address).
5. A statement of good-faith belief that the use is not authorized by the owner, its agent, or the law.
6. A statement, under penalty of perjury, that the information in your notice is accurate and that you are authorized to act for the owner.

2) Counter-Notification

If you believe removal or disabling was a mistake or misidentification, send a counter-notification including:

1. Your name, address, phone number, and signature.
2. Identification of the material removed and where it appeared before removal.
3. A statement, under penalty of perjury, of good-faith belief that the material was removed by mistake or misidentification.
4. A statement consenting to the jurisdiction of the U.S. federal court in your district (or, if outside the U.S., where the site operator may be found).

Send to the same email above with subject “DMCA Counter-Notification.”

3) Repeat Infringers

Accounts or access of repeat infringers may be terminated.

4) Reservation of Rights

This policy does not limit other rights or remedies under applicable law. Using a U.S. policy does not waive protections under Canadian law.
